Privacy Notice — Pilot Programme Application

This notice explains how we handle the information you send us when you apply for the Intuita pilot programme through the application form on this website. It covers only the application form; if you later use the Intuita service, the Privacy Policy applies to that use.

Who is responsible

Reza Labs, the sole proprietorship of Mohammadreza Khosravivala, registered in the Netherlands (KVK 42116766). Contact: reza@intuita.health.

What we collect

Only what you enter in the form: your name, work email address, and phone number; your professional profile (role, practice type, practice size, weekly session volume, country); anything you choose to write in the free-text fields; and your language preference with the time of submission.

Please do not include any information about your clients. The form is about you and your practice. The application is not the place for clinical information of any kind.

Why, and on what legal basis

We use your application to assess it, to contact you about it, and to plan and set up your participation if you are selected. The legal basis is Article 6(1)(b) GDPR: steps taken at your request prior to entering into an agreement. Providing the information is voluntary — without it, we simply cannot process an application.

We do not use your application for anything else: no advertising lists, no profiling, no automated decision-making, no sharing with third parties for their own purposes.

Where it is processed

Applications are stored on our servers in the European Union. When you submit the form, a notification is also delivered to our team through our email delivery provider, which is based in the United States; that transfer is safeguarded by the EU-U.S. Data Privacy Framework, with EU Standard Contractual Clauses as a fallback.

How long we keep it

We keep applications only as long as they are needed to run and follow up on the pilot programme selection, and delete them when they are no longer needed. You can ask us to delete your application at any time — doing so has no consequence other than that we can no longer consider it.

Your rights

You may ask us at any time to access, correct, or delete the information from your application, to restrict or object to its use, or to receive a copy of it. Write to reza@intuita.health — we respond within one month. You also have the right to lodge a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens) or your local supervisory authority.
